Fix Missing User Input Data

A particular piece of text that users should be able to enter is showing up as “NA” (not available) throughout my system instead of displaying or saving the actual value. This is strictly a textual data point collected through a user-facing form, not a numeric or geographic field.

I need you to:

• trace where the value disappears in the submission flow (front-end form, validation, API, database, or display layer);
• correct the root cause so the real text is captured and shown consistently;
• verify the fix with at least a few live test entries and provide a concise summary of what was changed.

The stack is a standard web setup—HTML/CSS/JavaScript on the front end with a typical server-side language (happy to share details once we connect). Please keep changes well-commented so I can maintain them later.
