Editable Word Paystub Template

I need a clean, reusable pay-stub template built in Microsoft Word (.docx). The base layout should carry the usual company and employee details, but I also want an intermediate level of customisation: clearly labelled areas for tax deductions as well as a separate line for bonuses. Everything has to fit neatly on a single letter-size page and remain fully editable so I can update names, dates and figures each pay period without breaking the formatting.

Key elements I must see
