Digital & Printable Editable Nametag

I need a reusable nametag template where only two elements—the person’s Name and Job Title—can be typed in each time. The finished badge must work in both scenarios: it should print cleanly on standard badge stock and also export neatly for online use (PDF or image).

I’ll be doing the edits on a Windows desktop, so please build the file in a format I can open without paid software—an editable PDF with form fields, a Word or PowerPoint template, or anything equally straightforward.

Deliverables
• One Windows-friendly template with clearly editable Name and Title fields
• A ready-to-print PDF that preserves those fields
• Brief usage notes showing exactly where and how to type the two pieces of text

Acceptance criteria
The layout, fonts, and colors stay locked while only the Name and Title can be changed; the printed version fits common badge holders; and the file opens on a standard Windows PC using free software such as Adobe Acrobat Reader or Microsoft Office.
