I need a seasoned medical writer to create clear, accurate treatment plans that can be uploaded directly into our EHR. Each plan must provide step-by-step clinical instructions rather than just a high-level summary, so practitioners can follow them without additional clarification.
You will receive a brief dossier for every case (diagnosis, current medications, allergies, recent labs). From that, I expect you to craft:
• A concise statement of the primary condition
• Evidence-based therapeutic objectives
• Medication, dosage, and titration details with ICD-10 and CPT references where appropriate
• Monitoring schedules, follow-up intervals, and red-flag warning signs for escalation
• Patient education notes formatted for portal release
I am focused solely on treatment plans right now, though familiarity with patient history compilation or billing terminology would be an advantage when cross-referencing codes.
Please submit a sample plan or a redacted excerpt you have authored so I can judge clinical tone, depth of instruction, and APA/AMA citation accuracy. Final files should arrive in both DOCX and PDF, ready for direct import into Epic or Cerner.
