Data Entry From Paper Reports -- 2

I have between 100 and 500 paper-based reports that need to be keyed into a digital format. Each report contains several fixed fields (dates, names, numerical results and short notes) and I need every field reproduced exactly as it appears on the page—no omissions, no spelling changes.

You are free to use Excel, Google Sheets or another structured format you’re comfortable with, as long as the data can later be sorted and filtered. Accuracy matters more than speed; I will spot-check random samples against the originals before signing off.

Deliverables
• A single file containing one completed row per report
• Consistent field naming that matches the headings I will provide
• Completion report confirming total records captured and any illegible items flagged

Please factor in that everything you will be working from is on paper, so you’ll be typing from scanned images I’ll supply as high-resolution PDFs. Let me know if you have any questions about the layout before you start so we can avoid rework later on.
