Customized 3D Printed Model for New Home

Hi! I'm planning to build a new home and would like a 3D printed scale model of it. Before I order, I have a few questions:

1. I have floor plans, elevations, and a site plan (PDF). Can you work from those? Do you need a 3D/CAD file instead?
2. I'd like an exterior-interior model at about [1:100 / 1:50 / your recommendation]. What would the finished dimensions be? The roof should be a lid to see inside home.
3. Does your price include modeling windows, doors, roof pitch, porches, and a chimney, or are those simplified?
4. Is the model raw, sanded, or painted? I'd like it painted in my chosen colors.
5. Can you include a base plate with the lot outline and driveway?
6. Can you send a preview render or photos of the digital model for my approval before printing?
7. What is the total price including shipping, where do you ship from, and how long will it take to arrive?
8. How do you pack it so it doesn't break in transit, and how many revisions are included?

Could you also share photos of a few house models you've made before? Thanks!
