Customised QR Code for Company Brochure and fillable forms downloadable and usable in Shofity

I need a branded, scannable QR code that instantly linked to a htmil page with a button of download brochure and delivers our PDF brochure + Fillable forms when customers point their phones at it. The scan must trigger an immediate brochure download—no redirects—so the experience stays friction-free.

The brochure itself is ready; it focuses on our product and service details plus real-world case studies and testimonials. What I’m missing is the technical glue:

• A visually on-brand QR code (our colours, logo in the centre)
• A lightweight lead-capture form that pops up before or after the download so interested visitors can share their details
• Seamless integration with my Shopify store—ideally a simple snippet or app block I can drop on any page and in confirmation emails

Time is critical; I’m looking to have this live ASAP. If you’ve handled QR generation, PDF hosting, and Shopify embedding before, let’s get it done quickly and cleanly.
