Custom Online Survey Form

I want a clean, responsive survey form built to capture only one thing—people’s opinions and feedback. It should be quick to load, intuitive on mobile and desktop, and easy for me to view or export the results (CSV or Google Sheets is fine). You are free to work in Google Forms, Typeform, Jotform, or a comparable tool as long as the final link can be embedded on my site and shared directly with respondents.
