Custom Fillable PDF Form

I need an editable PDF built from a sample document that users can complete digitally. The document will be used primarily for form filling, so every field must be fully interactive in Adobe Acrobat and the common browser viewers.

Core requirements
• Text fields for standard information entry (name, date, descriptions, etc.).
• Checkboxes for quick yes/no or multi-choice selections.
• A dedicated page at the end that shows a clean grid background so the user can sketch a diagram directly on the PDF. The grid should print sharply and sit behind whatever they draw.

I will supply the static content and any branding assets you need; you handle the layout, field tagging, tab order, and basic validation where appropriate. Please ensure the final file is unlocked for future edits and that all interactive elements remain intact when the PDF is flattened for archiving.

Deliverable
One ready-to-use, fillable PDF that meets the above specs, plus the working source file (InDesign, Illustrator, or similar) for any future revisions.

Acceptance
The file must open without warnings, every field must be editable, and the grid-based sketch page must allow free drawing while keeping the grid visible underneath.
