Convert Form Boxes to Visio

I have a paper / PDF form that contains roughly 150 individual boxes, each holding a short text-and-number label (e.g., “Section 12-A”, “Qty 25”). I need every one of those boxes recreated in Microsoft Visio as clean, uniformly sized rectangles. All copy inside the rectangles must appear in standard with some bold so it remains legible when the diagram is printed or zoomed.

Source material: I will supply a high-resolution scan of the form so you can see exact wording, order, and spacing.

What I expect back:
• A single .vsdx file containing the full layout, with each box accurately positioned, sized to match the original, and filled with the correct standard and bolded text-and-number label.
• The drawing should stay editable—no flattened images—so I can tweak positioning later if needed.
