I’m running Formidable Forms on WordPress and need a set of contact-style forms fully configured. The forms themselves are simple for the visitor—collect Name and Email—yet each submission must also carry four internal data points: Name, Email, ProductID, PaymentDate and PaymentStatus.

I need to be able to access these data from API I am developing, so I need to access these data by querying database.

Stripe will handle the payment step. After the user hits “Submit,” the form should initiate a Stripe checkout, record the PaymentDate automatically once Stripe confirms, then mark the PaymentStatus field as Paid (or Failed if the transaction is declined). I’d like the form to prevent submission if payment isn’t successful.

Deliverables
• One ready-to-embed Formidable form that captures Name, Email, ProductID, PaymentDate, PaymentStatus
• Stripe integration set up and tested in both test and live modes
• Automated status update and email notification on successful payment

Please keep the build inside Formidable Forms—no external plugins unless absolutely required—and document any hooks or custom code you add so I can maintain it later.
