I need a single, flowing record that tracks every physician I have seen, each diagnosis they offered, and the associated diagnostic explanation, arranged strictly in chronological order. This narrative should read like a story of my care, letting any clinician trace the full course of events without having to flip between separate files.

What to include
• Exact visit dates, doctor names, and locations
• All diagnoses issued at each encounter
• Diagnostic detail woven into the text, with my chosen emphasis on test results and imaging reports—please quote key figures, findings, and impressions verbatim where possible
• Clear links between a diagnosis and the tests or images that supported it

Format
My preference is a diagnostic-style document: something that still feels clinical yet is easy to follow. A well-structured Word or PDF file works perfectly as the final deliverable.

Quality requirements
