Responsibilities
Position Objective:
The Clinical Pharmacist is an operationarole,responsible for providing medication management services for our medicallyl and socially complex patients. The incumbent will report to the Manager of Pharmacy Services (or similar role) and will be responsible for managing opportunities for professional development within the pharmacy department.
Responsibilities:

Develop and maintain protocols for prescribing, dispensing, and administering medications

Conduct research to develop or improve pharmaceutical treatments

Provide specialized services to help patients manage conditions such as Diabetes, Chronic Obstructive Pulmonary Disease, Congestive Heart Failure, Asthma, or Hypertension

Contribute to the efficient delivery of care by participating in quality improvement initiatives

Participate in the development or revision of pharmacy policies and procedures.

Provide training and support to pharmacy staff and interns

Maintain up-to-date knowledge of new drugs, medical devices, and therapies available in the market

Ensure compliance with all federal, state, and local regulations related to pharmaceuticals

Consult with physicians on matters pertaining to drug prescribing practices

Participate in patient intervention and referral, risk management, and ethical decision-making

Actively participate in multidisciplinary team meetings to contribute to the overall patient care plan

Collaborate with healthcare professionals to plan, monitor, review, and evaluate patient effectiveness

Educate patients about medications, side effects, interactions, and therapeutic effects

Ensure medication safety and efficacy by reviewing patient profiles

Participate in patient care rounds and perform drug therapy consultations as required

Additional duties as assigned

Position Requirements/ Skills:

A doctor of pharmacy degree from an accredited institution

Licensure as a pharmacist in the state of practice

3 years of experience in a pharmacy setting

Proven experience as a clinical pharmacist

Excellent understanding of pharmaceutical therapy and direct patient care

Ability to manage multiple priorities in an expedient and decisive manner

Proficient with Google Suite (Drive, Docs, Sheets, Slides) and Microsoft Office (Word, Excel, PowerPoint) for real-time collaboration

Physical Requirements:

Requires standing, walking, pushing, bending, kneeling, and reaching

Ability to sit for extended periods of time

Requires corrected vision and hearing to normal range

Ability to operate a motor vehicle and have own means of transportation

Pay Range
