About Us
One in three people die of heart disease - it's time to change that. We're redesigning heart health from the ground up so that everyone can live fuller lives. Our team consists of mission-driven clinicians, engineers, and professionals attacking a problem using evidence-based research and guidelines for cardiovascular rehabilitation. We're working to deliver exercise and wellness to older adults with cardiovascular disease using telemedicine. We are dedicated to delivering exceptional services that enhance the lives of our patients.
Job Summary
The Clinical Eligibility & Intake Specialist splits time between two kinds of work. Most days, you will review the EMR records of cardiac recovery patients to determine medical eligibility for Aviary Health's virtual cardiac rehab program. When intake needs extra capacity, such as a last-minute call-out, a schedule gap, or a high-volume week, you will step in to run initial assessments with new patients over video.
This role suits a nurse who is equally comfortable working a chart review queue independently and sitting with a new patient to explain the program, review their history, and build their plan.
Responsibilities
Clinical eligibility (majority of hours)
Reviewing patient charts in Epic and other EMRs to determine medical eligibility/ineligibility using the inclusion/exclusion list and decision tree
Identifying current cardiac rehab referral orders, or creating cardiac rehab referral orders when needed
Communicating eligibility and ineligibility decisions to the enrollment team through concise documentation in the appropriate platforms
Working the eligibility queue within team turnaround targets so patients can be scheduled without delay
Completing eligibility rechecks and supporting special projects as needed
Escalating cases that fall outside the decision tree to the Clinical Eligibility Lead
Intake coverage (as needed, often same day)
Conducting initial assessments by video for patients enrolling in our virtual cardiac recovery program
Reviewing medical history, cardiac procedures, and medications before and during the visit
Completing individualized treatment plans that include nutrition, psychosocial, fitness, and co-morbidities assessments
Educating patients on program benefits and the importance of risk factor modification
Using goal setting and motivational interviewing to start patients off motivated and engaged
Documenting each visit accurately and on time per clinical documentation standards
This position requires the ability to learn and retain a broad base of clinical knowledge, switch between chart review and patient-facing work within the same shift, and make quick, independent decisions using good judgment.
Qualifications
Active RN or LPN license (required); multistate/compact license preferred
3-5 years of clinical experience in cardiac care, cardiac rehab, or chronic disease management
3 years of experience in Epic or another EMR, including chart review
Working knowledge of cardiac conditions, procedures, and medications, including CHF
Experience assessing and educating patients, ideally older adults, in a clinical or virtual care setting
Comfort with motivational interviewing and goal setting
Strong written communication, with concise and accurate clinical documentation
Proficient computer skills, including Zoom, Google Suite, and EMR systems
Fluent in English (written and verbal)
Bonus points
Bilingual in English and Spanish
Cardiac rehab experience or knowledge of cardiac rehab eligibility criteria
Virtual care or remote/1099 contract experience
Coaching experience in smoking cessation, diabetes management, or special populations
Requirements

Reliable internet connection and a quiet, private workspace suitable for live video visits with patients
SOPs
Clinical eligibility check process
Inclusion/exclusion list

Initial assessment and treatment plan documentation
Note: This is a 1099 contractor position.
