Build Google Survey From Spreadsheet

I have an existing spreadsheet that lays out every question and its answer choices. I need that content turned into a clean Google Form built for survey use. Each item should appear as a multiple-choice question, and the form must be divided into clear sections so respondents can move through the topics naturally.

Deliverables:
• A shareable Google Form linked to my Google account
• All questions reproduced exactly as in the spreadsheet, using the multiple-choice format
• Logical section breaks that mirror the grouping in the source file
• Confirmation that responses will be recorded in the linked Google Sheet automatically
