CONVERT STATIC FORM SO A REPORT IS GENERATED DEPENDING ON ANSWERS
I currently have a static form that parents use to answer questions about risk factors for their child with Type 1 diabetes. Currently, I have to manually go back and look at user responses to decide which answers are important. To save time, I want to automate this process by doing the following:
1. Assign a paragraph response to user answers
For example, if the person chooses answer A in question #1, we assign a paragraph (which I'll write in Admin database) to address that specific answer. This paragraph will be added to the final report. If the person chooses answer B, a different paragraph will be assigned for that answer. Not every answer will need to have a paragraph assigned - only answers that show a definite risk factor.
2. Set a Priority Level for each answer.
In the final Report, there will be two sections. The first will be labeled "AREAS OF CONCERN" and the second labeled AREAS OF CONSIDERATION. Some of my assigned paragraph responses will go in the first section, and others in the second. I will determine this by assigning each question a Priority Level, such as PL1, PL2, PL3, PL4, or PL5. If the answer is assigned PL1, PL2 or PL3, the associated paragraph for that answer will go into the first AREAS OF CONCERN section. PL1 paragraphs will appear first, PL2 paragraphs next, and PL3, PL4, and PL5 after that. If an answer is labeled PL0, it is put into the AREAS FOR CONSIDERATION section. If a Priority Level is labeled as NONE, there is no paragraph attached so it won't be mentioned in the report.
DATABASE ORGANIZATION
In the admin database, I will need to be able to add and modify questions and answers (as seen in the current form at link below). The first text box will be for adding the question. Below that will be a text box to add multiple-choice answers. Next to each answer will be a way to assign a Priority Level for that answer. We then have to create a way I can assign a specific text box paragraph to each answer assigned PL1 through PL5 and PL0.
3. Summary Report
Upon completion, A button will appear that says - View Summary Report. Upon clicking that, a title at the top of the summary report will read "T1D Environmental Survey." Below that it will show the Date, Child's name, Child's age and maybe a few other things. There will also be a box that with the title "T1 Risk Factor":
If a person has many P1 answers this is a major risk factor for T1 Diabetes, so I want the user to be made aware of that. So, along with each question having its own assigned paragraph and rating of P1, P2, P3, P4, P5, or null, we are going to give a point value weight to their answers.
PL1=5 points
PL2=4 points
PL3=3 points
PL4=2 points
PL5=1 point
The program will add up these numbers and display the total in a box on the summary page. Above the box will be the title T1 Risk Factor." Any questions let me know - thanks!
