Automate Google Form Survey

I have a spreadsheet that already holds every question and option I need; now I want that data turned into a live Google Form survey. The form will be built entirely from multiple-choice items—well over ten of them—so no short-answer fields or rating scales are required.

All I need back is:

• A Google Form that mirrors my sheet exactly (question wording, option order, section titles if any).
• Confirmation that each response funnels into its linked response sheet without formatting issues.

Feel free to use Apps Script, the Forms API, or any other reliable method, as long as the finished form is delivered ready to send out.
