Android Survey Form Builder

I’m putting together a simple yet reliable survey form that will run natively on Android phones. All questions will be straightforward text-input fields, so users can type their responses freely rather than tapping radio buttons or sliders. I also need each respondent to be able to attach a file—most often a photo or PDF—before submitting the form.

A clean Material-style interface, smooth validation, and quick submission feedback are important to me; lag or crashes are not an option. The finished work should include both the installable APK for immediate testing and the full Android Studio source so I can extend the question set later if needed. If you prefer Kotlin or Java, either way works—just keep the code well-commented.

Once everything is running, I’ll verify that:
• Every text field captures and stores exactly what the user enters.
• The file-upload element accepts common formats and pushes them with the rest of the form data.
• Submission succeeds over typical mobile connections without data loss.
